Therapy and Consultation: $170.00 for 45 minute session, $190 for 60 minute session
Assessment and Evaluations: $200.00 per hour
Cancellation Policy: Dr. da Silva requires a 24 hour cancellation in writing (via email). This allows for Dr. da Silva to offer your appointment time to other individuals who are on a waiting list or in need of rescheduling their appointment. If you do not cancel within the 24 time period, you will be charged in full for your session.
Insurance: At this time, Dr. da Silva is considered an out-of-network provider for insurance providers. For out-of-network companies, client's are provided with monthly statements that they may submit to their insurance companies for reimbursement. It is the client's responsibility to consult with their insurance companies about their out-of-network coverage. Fees are due at the time of the appointment.
